The following link is the initial application for the Payer Sponsored EMR Adoption Program. Please download the form, complete it and either e-mail it to mcosentino@wnyhealthelink.com or mail it to HEALTHeLINK, The Commons at Walden, 2568 Walden Avenue, Suite 107, Buffalo, NY 14225. Your completed application will be reviewed against the eligibility criteria within two to three weeks of submission at which time you will be notified as to the disposition of the review. The application is the first step of the approval process. If your application is accepted the next step will be a formal readiness assessment to determine your practices readiness to implement an EMR.
